What Is IVF?

IVF stands for in vitro fertilisation.

Very simply, eggs are collected from the ovaries and fertilised with sperm in a laboratory. The fertilised egg, now called an embryo, is then transferred into the uterus.

The explanation sounds simple, but IVF is not one isolated procedure. It is a carefully managed series of steps completed over several weeks.

During this time:

  • Medication stimulates the development of eggs
  • Ultrasound scans monitor egg growth
  • Eggs are retrieved from the ovaries
  • Sperm is prepared in the laboratory
  • Fertilisation takes place

The laboratory stage is central to the process. Once eggs have been retrieved, they may remain in the IVF laboratory for six or seven days while embryologists assess fertilisation and embryo development.

 

IVF was originally developed for women with blocked or damaged fallopian tubes.

The fallopian tubes normally allow the egg and sperm to meet. If the tubes are blocked or damaged, natural fertilisation cannot take place. IVF bypasses this problem by bringing the egg and sperm together in a laboratory.

What Happens During an IVF Cycle?

A typical IVF cycle spans around 28 days.

The first day of the cycle is the first day of the patient’s menstrual period. Two or three days later, fertility medication is started to stimulate egg growth.

A scan is usually performed approximately one week later. Further scans allow the fertility team to monitor the eggs and determine when they are ready for retrieval.

If the eggs are ready, a trigger injection prepares the patient for egg retrieval. The eggs are then collected and transferred to the IVF laboratory.

On egg retrieval day:

  • The embryologist receives and assesses the eggs.
  • The sperm is prepared.
  • Fertilisation takes place in the laboratory.
  • The embryo(s) are nurtured for several days.
  • The best available embryo or embryos are selected.
  • The selected embryo is prepared for transfer into the uterus.

How Embryologists Prepare Sperm for Fertilisation

The sperm used during treatment may come from the patient’s partner or from a donor.

Once the sample reaches the laboratory, the embryology team processes or “washes” it. This preparation helps isolate the sperm that will be used for fertilisation.

The way fertilisation takes place depends on the treatment plan.

ICSI gives the embryologist direct control over introducing a selected sperm cell into a mature egg.

 

How ICSI Supports Fertilisation

During ICSI, an embryologist injects an individual sperm directly into a mature egg.

  • High-powered microscopes

  • Micromanipulation equipment

The procedure is highly precise. The embryologist must select and handle the sperm and egg carefully throughout the process.

After the sperm has been injected, the egg is placed into a specially designed culture medium. This medium is covered with oil and is designed to mimic the fluid found in the fallopian tubes.

It contains proteins, amino acids and enzymes that support embryo development.

The egg is then placed in a controlled incubator, where the embryology team can continue monitoring it.

 

How Embryos Are Nurtured and Monitored

The day after ICSI, the embryologists examine the eggs to check whether fertilisation has occurred.

The team then continues to monitor the embryos as they develop.

An embryo changes quickly during its first few days. Embryologists assess this development and record the information needed to compare the embryos.

How Embryos Are Selected for Transfer

The embryology team collects information about each embryo so that the best available embryo or embryos can be considered for transfer.

Selection is not based on one quick observation. It follows several days of monitoring and assessment.

Once the fertility specialist and patient have discussed the results, the embryologist prepares the selected embryo for transfer.

The embryo is carefully loaded into a catheter. The fertility specialist then uses the catheter to transfer the embryo into the uterus.

Although the transfer itself is performed by the fertility specialist, the embryologist’s preparation is essential. The correct embryo must be selected, identified and loaded safely.

 

The Role of Embryologists in Freezing

Embryologists are also responsible for preparing eggs, sperm and embryos for freezing when required.

Freezing may form part of:
