Why Sperm Testing Matters
Male infertility can result from many different factors.
Some possible causes are easy to recognise, such as a previous vasectomy or spinal cord injury. Others are less obvious. What Is Sperm Testing?

The first male infertility test we usually perform at Cape Fertility is a routine semen analysis. This test helps determine whether the sperm sample is compatible with fertility and whether pregnancy can reasonably be expected.
The semen analysis does not simply provide one pass or fail result. It measures several parts of the sample because sperm quality depends on more than sperm count alone.
For example, a sample may contain a reasonable number of sperm, but the sperm may not move effectively. Another sample may have good movement but a low volume. In other cases, the routine results may appear normal even though a deeper sperm function problem exists.
Each result must therefore be considered as part of the full report and your broader medical assessment.
Who Should Consider a Sperm Test?
Everyone should consider sperm tests as it is the only way to assess male fertility.
It may also be useful where there are possible risk factors for male infertility, including:
- A previous vasectomy
- A spinal cord injury
- Medical conditions affecting fertility

- Steroid or testosterone use

What Does a Routine Semen Analysis Check?
A semen analysis examines several connected factors.
There is no single measurement that tells the entire story. A specialist needs to understand how the different results work together.
The main areas assessed are:

- Morphology
- pH
- Vitality

Understanding Semen Volume
Semen volume refers to the amount of fluid in the full sample.
A normal volume is at least 1.5 ml.
If the volume is much lower, it may raise concern about a possible obstruction somewhere between sperm production and ejaculation.
The volume result does not confirm an obstruction on its own. It gives the fertility specialist information that may need to be considered alongside your medical history and the rest of the semen analysis.
Understanding Sperm Concentration
Sperm concentration measures how many sperm are present in each millilitre of the sample.
The laboratory also considers the total number of sperm in the complete sample.
For example, if the full semen sample measures 3 ml, the laboratory may assess the number of sperm present in 1 ml and use this information alongside the total volume.
The testes produce millions of sperm. Around 15 million sperm is generally regarded as normal within the context of Cape Fertility’s routine analysis.
A lower result does not automatically mean pregnancy is impossible. It indicates that your fertility specialist needs to examine the result together with motility, morphology and the other measurements.
Understanding Sperm Motility
Motility describes sperm movement.
The semen analysis looks at both general motility and progressive motility.
General motility
General motility refers to the percentage of sperm that are moving at all. Some may only twitch. Others may swim in circles. These sperm are moving, but they may not be making useful forward progress.
Around 42% total motility is considered within the normal range.
Progressive motility
Progressive motility looks at the sperm that move forward from one point to another.
This matters because sperm need to move effectively to reach and fertilise an egg.
Around 32% rapidly progressive sperm is considered within the normal range.
A sample can therefore contain moving sperm without all of those sperm travelling forward properly. Testing both measurements gives the fertility team a more accurate understanding of sperm function.
Understanding Sperm Morphology
Morphology refers to the shape and appearance of sperm.
Men are often concerned when they hear that only 4% or 5% of their sperm appear normal. This number may sound very low, but the testes produce millions of sperm and errors in appearance are common.
Only a small proportion of sperm may need to appear normal for the morphology measurement to fall within the expected range.
Morphology should not be interpreted alone. The number of sperm, their movement and other test findings also matter.
A result showing 4% normally shaped sperm does not automatically mean you cannot achieve a pregnancy.
Other Factors Checked During Sperm Testing
A routine semen analysis may also examine pH, vitality and antibodies.
Semen pH
The pH result can provide information about the ejaculatory ducts that produce the fluid supporting the sperm. This fluid forms part of the environment in which sperm must survive and move.
Sperm vitality
Vitality measures how many sperm in the sample are alive. Movement and vitality are connected but not identical. A sperm cell may not be moving, which makes it important to establish whether it is alive or not.
Antibodies
Antibodies may sometimes interfere with normal sperm movement. This can be particularly relevant where a man has previously undergone a vasectomy and later had a vasectomy reversal.
The semen analysis report allows the specialist to assess these different factors together rather than making a decision based only on sperm count.
