The IVF procedure was pioneered in the United Kingdom in 1978 by Sir Robert Edwards, together with embryologist Jean Purdy and gynaecologist Patrick Steptoe.

As early as 1958, Sir Robert recognised that fertilisation outside the body could help treat infertility. At the time, this idea faced strong criticism from religious leaders, ethicists and even parts of the medical community.

Despite years of obstacles and moments where the team nearly gave up, they continued their work, encouraged by letters from couples who desperately wanted children.

On 25 July 1978, the world’s first IVF baby, Louise Brown, was born. Since then, an estimated eight million babies have been born through IVF treatment worldwide.

Sir Robert Edwards later received the Nobel Prize in Physiology or Medicine for his work, recognising IVF as one of the most important medical advances in reproductive health.

IVF Explained Simply: How The Process Works

IVF may sound complex, but the concept itself is straightforward.

An egg is fertilised with sperm in a laboratory.

The embryo is placed into the uterus.

If implantation occurs, pregnancy begins.

What many people do not realise is that IVF is not one procedure. It is a series of carefully timed steps that usually take place over about 28 days.

From the first day of your cycle to the pregnancy test, each stage is designed to maximise your chance of success.


Stage 1: Preparing the body for IVF

IVF treatment usually begins on day one to three of your menstrual cycle.

The first stage involves medication to:

  • Stimulate the ovaries so multiple eggs can develop
  • Prevent ovulation from happening too early

These medications may be tablets or injections. Because hormone levels are changing, some women experience side effects, which is why careful monitoring is essential.

Monitoring typically includes:

  • Blood tests
  • Ultrasound scans starting around day eight
  • Follow-up scans on days ten and twelve

Stage 2: Egg collection

Once the eggs are ready, an injection of HCG is given about 36 hours before egg collection.

Egg collection:

  • Takes about two hours
  • Is performed under sedation
  • Uses a fine needle guided by ultrasound

After egg collection, progesterone is given to prepare the lining of the uterus for embryo transfer.


Stage 3: Fertilisation

On the same day as the egg collection:

  • A sperm sample from the male partner or a donor is prepared
  • Eggs are fertilised in the laboratory

This is where IVF truly happens.


Stage 4: Embryo development and selection

After fertilisation, embryos are grown in the laboratory for three to five days.

Thanks to advances in embryo culture:

  • Embryos can develop to the blastocyst stage on day five
  • Higher quality embryos can be selected for transfer

Any additional good-quality embryos are frozen and stored in liquid nitrogen. These frozen embryos can be used in future treatment cycles without repeating egg collection.


Stage 5: Embryo transfer and implantation

Embryo transfer is a simple and painless procedure.

  • No anaesthetic is required
  • An ultrasound guides placement into the uterus

After transfer:

  • The embryo may take six to seven days to implant
  • Medication may continue to support the uterine lining

Stage 6: Pregnancy test

A blood test is performed 14 days after egg collection to confirm pregnancy.
